Athletic Registration

 

1. Athletic Registration Packet MUST BE COMPLETED

2. Physical Form COMPLETED AND CLEARED by Physician for Participation (must be on athletic physical form)

3. Emergency & Insurance Information Verified (in folder)

5. Sudden Cardiac Arrest, Heat Illness Prevention, and Concussion Information Sheet signed and on file with school (in packet)

4. Bills and Fines CLEARED (RHS Finance Office)

5. Must have a 2.0 GPA on MOST RECENT progress report (Athletic Department confirms academic eligibility)

6. Return completed Packet and Folder to RHS Finance Office
